I am a qualified medical examiner for officers and mariners with a qualified rating as well as for entry level mariners. I would be happy to arrange to get your USCG medical exam completed the same or following day whenever possible.

I understand your concerns about the process and encourage you to call me with any health concerns or medications that might impact the issuance of your USCG medical certificate.

What to Bring to the Medical Examination

• California (or other state) driver license or other government issued photo ID.

• If you are an officer or mariner with a qualified rating, bring a copy of Form CG-719K, Report of Medical Examination , with all of the required information filled out by you.

• If you are an entry level mariner, bring a copy of Form CG-719K/E, Report of Medical Examination , with all of the required information filled out by you.
